Not Losing Weight on a Low- Carb Ketogenic Diet? Don’t Give Up and Read Further. The ketogenic diet is not only known to be one of the most effective weight loss tools, but has proven to have many health benefits. Ketosis is a state at which your body produces ketones in the liver, shifting the body's metabolism away from glucose and towards fat utilization. Unless you can check your blood ketones, using Ketostix is an easy way to detect urinary ketones. It's not the most accurate method, but may be good enough to find out whether you are in ketosis. In some cases, weight loss may be difficult even on a low- carb ketogenic diet and there may be a few possible reasons for weight stalling, which I have listed in this post. Top Reasons You Are Not Losing Weight on a Keto Diet. Carbs are Too High. Your carbohydrate intake may be too high. Try to decrease your daily carbs limit. Also try to include coconut oil in your diet. Coconut oil consists of MCTs (Medium chain triglycerides), which are easily digestible, less likely to be stored by your body and are used for immediate energy. MCTs are converted in the liver into ketones, which helps you enter ketosis. For more about ketones, have a look at this post. Protein is Too High or Too Low. Your protein intake may be too high/ low. Protein is the most sating macronutrient and you should include high- quality animal protein in your diet. If you don't eat enough protein, you will feel more hungry and most likely eat more. Lack of protein can also lead to muscle loss. However, if you eat too much protein, the excess protein converts into glycogen and disrupts ketosis. Keep in mind that you will have to eat a lot more than you should - 1. This is not the most likely reason, as it's not as easy to eat too much protein unless you take protein supplements. Phinney and Volek in their book . Carb Cheating / Carb Creep. Carb cheating / carb creep is another possible reason for weight loss plateaus. You have to be really disciplined and aware of all carbs you eat. It’s a little nibbling here and there of the forbidden foods, leading to a carb creep, so make sure you count all carbs. The Primal Blueprint Carbohydrate Curve. Yesterday, low- carb blogger Dr. Michael Eades (he of Protein Power) posted a message from his friend and fellow low- carb guru Richard Feinman as sort of a call- to- action in public policy- making for upcoming 2. USDA guidelines. Feinman have suggested that we ought to quickly find a way to help the USDA arrive at a sensible recommendation for carbohydrate consumption. Feinman asked: “how can the benefits of carbohydrate restriction that you have experienced personally or in your immediate environment be translated into reasonable recommendations that the USDA could put out?” In conjunction with my forthcoming book “The Primal Blueprint”, I have been working on an easy- to- understand explanation of how carbohydrates impact the human body and the degree to which we need them (or not) in our diet. A sugar- burner can’t even effectively access dietary fat for energy. As a result, more dietary fat is stored than burned. Unfortunately for them, they’re likely to end up gaining lots of body fat. As we know, a low ratio of fat to carbohydrate oxidation is a strong predictor of future weight gain. A sugar- burner depends on a perpetually- fleeting source of energy. Glucose is nice to burn when you need it, but you can’t really store very much of it on your person (unless you count snacks in pockets, or chipmunkesque cheek- stuffing). Even a 1. 60 pound person who’s visibly lean at 1. Depending on the nature of the physical activity, glycogen burning could be perfectly desirable and expected, but it’s precious, valuable stuff. If you’re able to power your efforts with fat for as long as possible, that gives you more glycogen – more rocket fuel for later, intenser efforts (like climbing a hill or grabbing that fourth quarter offensive rebound or running from a predator). Sugar- burners waste their glycogen on efforts that fat should be able to power. Being fat- adapted, then, looks and feels a little bit like the opposite of all that: A fat- burning beast can effectively burn stored fat for energy throughout the day. If you can handle missing meals and are able to go hours without getting ravenous and cranky (or craving carbs), you’re likely fat- adapted. A fat- burning beast is able to effectively oxidize dietary fat for energy. If you’re adapted, your post- prandial fat oxidation will be increased, and less dietary fat will be stored in adipose tissue. A fat- burning beast has plenty of accessible energy on hand, even if he or she is lean. If you’re adapted, the genes associated with lipid metabolism will be upregulated in your skeletal muscles. You will essentially reprogram your body. A fat- burning beast can rely more on fat for energy during exercise, sparing glycogen for when he or she really needs it. As I’ve discussed before, being able to mobilize and oxidize stored fat during exercise can reduce an athlete’s reliance on glycogen. This is the classic “train low, race high” phenomenon, and it can improve performance, save the glycogen for the truly intense segments of a session, and burn more body fat. If you can handle exercising without having to carb- load, you’re probably fat- adapted. If you can workout effectively in a fasted state, you’re definitely fat- adapted. Furthermore, a fat- burning beast will be able to burn glucose when necessary and/or available, whereas the opposite cannot be said for a sugar- burner. It means that a fat- burning beast will be able to handle some carbs along with some fat. A fat- burning beast will be able to empty glycogen stores through intense exercise, refill those stores, burn whatever dietary fat isn’t stored, and then easily access and oxidize the fat that is stored when it’s needed. It’s not that the fat- burning beast can’t burn glucose – because glucose is toxic in the blood, we’ll always preferentially burn it, store it, or otherwise “handle” it – it’s that he doesn’t depend on it. Once the fat- burning machinery has been established and programmed, you should be able to effortlessly switch between fuel sources as needed. There’s really no “fat- adaptation home test kit.” I suppose you could test your respiratory quotient, which is the ratio of carbon dioxide you produce to oxygen you consume. An RQ of 1+ indicates full glucose- burning; an RQ of 0. Somewhere around 0. The obese have higher RQs. Diabetics have higher RQs. Nighttime eaters have higher RQs (and lower lipid oxidation). What do these groups all have in common? Lower satiety, insistent hunger, impaired beta- oxidation of fat, increased carb cravings and intake – all hallmarks of the sugar- burner. It’d be great if you could monitor the efficiency of your mitochondria, including the waste products produced by their ATP manufacturing, perhaps with a really, really powerful microscope, but you’d have to know what you were looking for. And besides, although I like to think our “cellular power plants” resemble the power plant from the Simpsons, I’m pretty sure I’d be disappointed by reality. No, there’s no test to take, no simple thing to measure, no one number to track, no lab to order from your doctor. To find out if you’re fat- adapted, the most effective way is to ask yourself a few basic questions: Can you go three hours without eating? Is skipping a meal an exercise in futility and misery? Do you enjoy steady, even energy throughout the day? Are midday naps pleasurable indulgences, rather than necessary staples? Can you exercise without carb- loading? Fat- adaption describes the ability to burn both fat directly via beta- oxidation and glucose via glycolysis, while ketosis describes the use of fat- derived ketone bodies by tissues (like parts of the brain) that normally use glucose. A ketogenic diet “tells” your body that no or very little glucose is available in the environment. Why hemoglobin A1c is not a reliable markeri was recently tested for Hemoglobin A1c because i presented to an endocrinologist with extremely low blood glucose on lab test and some scary symptoms, not the ordinary hypoglycemia symptoms. My A1c was 4. 7 which registered as low (L) on the lab print out–it was only slightly low. Does a low score on this suggest a possibility of short- lived RBCs? Does it have any relationship with extremely low blood glucose? I soon found that i had very impaired coordination. Eventually i wanted to have breakfast but had great difficulty holding the measuring cup under the faucet, to get some water to heat, to make instant oatmeal, i lacked the coordination to get the water into the cup. I persisted and did make the instant oatmeal (pour hot water onto flakes and it’s done), and i got my lap top and was eating the oatmeal and i suddenly was aware that the symptoms were going away. Previously i had been unable to type. While eating the small amount of oatmeal, i realized i could type. That was about a month before the lab test. Since it only happened that once, i put it out of my mind. About 5 days after the lab test, i had the second episode, worse than the first, i woke falling out of bed to the floor, couldn’t use my arm to break the fall, i didn’t have the coordination. I have had some blood tests, a non fasting repeat of blood glucose was 6. A1c scoring 4. 7. More recently i’ve had another lab test which included non fasting glucose and insulin. Glucose was 5. 3 (i had eaten about an hour before the test, whole grain rye bread, tuna, apple), insulin was 5. I was wondering about the A1c and after reading this article, am wondering if this could be evidence that my RBCs are short- lived. I have no results showing anemia, most recent result in late Februarly, was 4. RBCs. Other CBC results were normal. After realizing i was hypoglycemic, i started eating frequent meals and i started feeling better and my head got clearer, but then after a couple of months, i was slacking off on the number of meals and i was going back to my previous habit of not eating breakfast when first arising but going for a 1/2 hour walk, doing some yoga, maybe straightening up the house. I began feeling less well and my glucose results on my home meter were getting lower and lower. Two weeks ago, i woke up one morning laying on my back on the floor by the foot of my bed, having no idea how i got there. The next morning it was similar. I woke laying on the floor next to my bed, no idea how i got there, unable to get up, stuck on my back, struggling to get up, no results, for about 1. Since then, i’ve been eating very frequently and was also eating during the night and before getting out of bed in the morning. I have been using a home glucometer since early March, i always have really low glucose in the morning. Once in a while i get a result int he 4. When i started eating a lot, including during the night, the results on the meter began increasing, the 7 day average was in the 6. But then, i stopped eating during the night, it seems very unhealthy to me, i hate doing it and can’t sleep, and i don’t know if that is why but my numbers have been going down. The doctor who saw me on Monday and referred me to another endocrinologist described me as pale and fatigued (he was recording his comments, like chart notes). I saw my Chinese doctor yesterday and she looked very worried about me, she said i was pale and she emailed me this morning and asked how i was and told me she wanted me to come in for a treatment. The GI doctor that the first endocrinologist sent me to said he thought it was odd that no one had taken insulin levels and said that insulinoma should be ruled out based on the history. So, he ordered blood tests that included glucose and insulin, he referred me to the second endocrinologist who i will see in 5 days, the earliest appointment i could get, despite reporting my symptoms. I want to know what testing i should ask for to help diagnose what is going on. |
